How does an employee sign up for the Louisiana Care Fund?
In order to distribute money as quickly as possible, the LHAREF will
work with Louisiana hospital leadership to identify employees who meet
eligibility criteria for the fund. The LHAREF sent instructions
and forms to hospital leadership on Sept. 16. Hospital leadership can contact LouisianaCareFund@lhaonline.org to request the application/distribution packet.
Where should a hospital employee submit the fund application (Form A)?
A hospital employee should submit the completed fund application (Form A) to his/her HR department, hospital administration or the individual handling the Louisiana Care Fund at the hospital. Hospital employees SHOULD NOT submit Form A directly to the LHAREF/Louisiana Care Fund Staff.
Who should the employee have sign as a witness on the fund application (Form A)?
An employee should have his/her HR department, hospital administration or the individual handling the Louisiana Care Fund at the hospital sign as the witness on his/her fund application (Form A).
How will the funds be distributed?
Hospitals will help identify employees who meet the following criteria:
The LHAREF will send a check from the Louisiana Care Fund payable to qualifying hospitals based on the number of eligible employees submitted from each facility. Hospitals will then distribute the funds to the designated employees.
How did the Louisiana Care Fund determine its distribution policy?
The goal of the policy is to ensure that the funds donated to the Louisiana Care Fund are:
- Used in the most appropriate and effective manner to assist hospital employees who experienced significant property loss or personal hardship as a result of Hurricane Laura or
- Distributed in a timely manner with a minimum amount of paperwork; and
- Distributed in their entirety with no administrative costs deducted.
How will I know that the Louisiana Care Fund received my donation?
Donors will receive a written or electronic acknowledgement that will include information needed for tax purposes.
How much of my donation goes to the Louisiana Care Fund?
All funds will be used to directly assist hospital employees who experienced significant property loss or personal hardship as a result of Hurricane Laura or
All administrative services are being provided in kind so that 100 percent of donations will be used to assist hospital employees.
Can I donate to a specific hospital or employee?
The Louisiana Care Fund cannot process distributions to designated individual hospitals or staff.
Information submitted by donors will not be sold, leased or otherwise disclosed to third parties other than is necessary to complete your transaction. The Louisiana Care Fund will only communicate with you to acknowledge receipt of your donation or resolve a problem with your transaction.
Can hospital employees impacted by Hurricane Delta apply?
Yes. Hospital employees affected by Hurricane Delta may apply as long as they meet the criteria.
If I was impacted by Hurricane Laura and Hurricane Delta, can I submit two separate forms?
No. Individuals can only submit one “Employee Request for Assistance” form (Form A).
